This form is used for all WA Seniors Card membership updates and requests. It can be used to apply for a new or replacement WA Seniors Card, update your contact or personal details, change your name, advise of a marital or spouse status change, nominate a Power of Attorney, apply for the Cost of Living Rebate, update your bank details, or notify us of the passing of a WA Seniors Card member. Completed forms may be submitted by email or post.
